Section 1 of 6Overview

Overview

Enicepatide is the INN for an investigational once-weekly dual GLP-1 / GIP receptor agonist. Discovery used Carmot Therapeutics' code CT-388; Roche/Genentech list RO7795068 and RG6640. Clinical protocols use subcutaneous once-weekly dosing. Peer-reviewed work describes cAMP-biased signalling with minimal β-arrestin recruitment / receptor internalization relative to some incretin benchmarks.

Carmot led early-phase work; Hoffmann-La Roche / Genentech lead phase 3. No FDA or EMA approval was identified as of the 2026-10-07 dossier cut.

Identity soft fields

No PubChem CID was securely identified in the dossier search. CAS 2869147-44-2 and UNII MTP9QR7GYF appear in GSRS/secondary registers — treat as reported identifiers pending GSRS/PubChem verification, not as hard-locked chemistry until confirmed. Legacy URL ct-388 permanently redirects here.

Same receptor axis, different molecules

Tirzepatide is an approved dual GLP-1/GIP agonist. VK2735 is another investigational dual on the same axis. Enicepatide is a separate molecule and sponsor lineage. There is no published head-to-head versus tirzepatide.

Section 3 of 6Key human results

Key human results

Phase 1 — peer-reviewed (PMID 41319798; NCT04838405)

Published Molecular Metabolism report of SC single doses in a 0.5–7.5 mg range or four once-weekly doses in a 5–12 mg range. Day-29 weight change was about −4.7% to −8.0% across doses versus about −0.5% placebo; PK supported once-weekly dosing. Adverse events were described as consistent with other incretin therapies (mostly mild/moderate).

A Roche May 2024 investor communication cited about 18.8% placebo-adjusted weight loss at 24 weeks from a phase 1 extension — mark as company ad-hoc until the same estimand appears in peer-reviewed full text (PMID 41319798 primarily covers the earlier MAD window).

Phase 2 obesity CT388-103 — Roche IR topline (NCT06525935)

Roche's 27 January 2026 investor update reported once-weekly CT-388 titrated up to 24 mg, with 22.5% placebo-adjusted weight loss (efficacy estimand) at 48 weeks, dose-response, and no plateau reported in the release. Treatment-regimen estimands were also mentioned — cite the IR and keep estimands distinct.

Phase 2 22.5% figure is press topline

Until a peer-reviewed full phase 2 paper is available, treat the 22.5% @ 48 weeks figure as sponsor topline (Roche IR 2026-01-27), not as an established peer-reviewed effect size.

Phase 2 T2D CT388-104 and phase 3 Enith

CT388-104 (NCT06628362) was marked completed on ClinicalTrials.gov; require primary peer review or posted results before claiming HbA1c/weight magnitudes. Enith1 and Enith2 were recruiting; outcomes were not available.

ZYNERGY combination with petrelintide — no efficacy yet

NCT07589686 is a phase 2 dose-finding study of petrelintide co-administered with enicepatide versus monotherapies versus placebo. Status at the dossier cut: NOT_YET_RECRUITING. Zealand/Roche collaboration materials describe early fixed-dose-combination exploration. No published combination efficacy exists — cross-link only; do not merge effect sizes into either monotherapy monograph.
